Submit an Incident Report

If you have been involved in an incident, you must submit the form linked below within 24 hours of the incident occurring, or at the earliest opportunity if 24 hours have already elapsed.

An incident is any occurrence that results in or has the potential to result in:

  • illness
  • injury
  • damage to property, equipment, material, vehicles, or the environment.
  • Issues relating to a Social Carer or Member's overall safety and well-being.

Where incidents have occurred as a result of emergency situations, always seek assistance from emergency services by calling 000 as your first priority.
